Our Customers

We work hand in hand with health departments, health plans, FQHCs & CHCs, rural providers, hospitals, primary & specialty care, behavioral health, and correctional facilities.

Health Departments

Our work with health departments gives underserved populations access to quality, whole-person care at a fraction of the cost.

Customer: LA County Department of Health Services

Challenges

  • Inconsistent referral protocols
  • Medically unnecessary specialty referrals
  • Long wait times for specialty visits (up to 12 months)
  • Patient transportation barriers
  • Lack of coordination between PCPs & specialists

Outcomes

  • 46% of electronic consultations resolved without a face-to-face specialist
  • 60% reduction in the average wait time for a specialty appointment
  • Dramatic improvements in communication & care coordination

The system is a win-win-win for LADHS. For our patients it means better access to specialty care, often without having to travel. For our primary care providers, it means rapid and direct communication with the specialist and updates on the best way to treat difficult problems. For our specialists, it means seeing the patients for whom they can add the most value.

Mitchell Katz, M.D., Director of LADHS

Behavioral Health

Our solutions enable primary care physicians to provide behavioral health assessments in the medical home.

Customer: L.A. Care Health Plan & L.A. County Department of Mental Health

Challenges

  • Insufficient care coordination between PCPs and mental health providers
  • Limited access to behavioral health services
  • Lack of behavioral health referral guidelines

Outcomes

  • Direct, electronic communication between LADMH and over 200 PCPs
  • Increased access and care coordination
  • Early identification and treatment of behavioral health needs
  • Referral prioritization for at-risk individuals

Mental and physical health are inextricably linked - to be successful we must provide a holistic and closely coordinated approach to care management. With this technology we can help facilitate the communication necessary for better care, while reducing barriers for primary and specialty care physicians - and for their patients.

John Baackes, CEO of L.A. Care Health Plan

Rural Providers

Our solutions give rural providers direct connection with remote specialists to provide timely specialty advice for patients.

  • Specialists often located hundreds of miles away

Outcomes

  • Timely specialty care delivered within the primary care office
  • Integrated televideo services for enhanced specialty network
  • Increased health equity for northern California's underserved

The proof is in the numbers. SNC's technology provides the ability to make positive changes in the health and wellbeing of thousands of rural northern Californians in the convenience of their primary care offices, saving valuable time and resources.

Psychiatric Facilities

Our solutions help psychiatric facilities provide on-campus specialty services for mental health patients.

Customer: California Department of State Hospitals - Metropolitan (DSH-Metropolitan)

Challenges

  • DSH-Metropolitan houses an aging population with many specialty care needs
  • Difficult to obtain specialty services for state hospital patients at county medical centers
  • Over-utilization of the ER for non-emergent issues like dermatology, endocrinology, or gastroenterology

Outcomes

  • Convenient online connection between DSH-Metropolitan staff and LAC + USC Medical Center specialists
  • Specialty issues resolved on campus, alleviating stress, expense, and liability associated with patient transport
  • Efficiency, cost reduction, and appropriate medical care

At DSH-Metropolitan, we house an aging population of chronically severely ill patients. Our 60-bed skilled nursing facility is always full, and there is always a waiting list. Converge gives us a direct link to specialty care - it's like being right there on the LAC + USC campus. It eliminates the need to transport patients for an initial evaluation, and often provides specialty care without the need to even leave the building at all, which saves time, effort, and expense.

Dr. Michael Barsom, Executive Director of DSH-Metropolitan
